  • Filled up application form for foreign exchange for medical treatment abroad [GFET appendix 5/83].
  • Recommendations from the medical board set up by the Health Directorate or appropriate medical specialists.
  • Cost estimate for the foreign medical institution.
  • Valid passport with all expired passports.
  • Filled up Medical Quota Debit Card Application Form.
  • Filled up Debit Card Endorsement Form.
  • Customers must submit original medical/hospital bills/vouchers once they arrive in Bangladesh from abroad (within 15 days of arrival).
